    TO GET A MEDICAL CARD, APPLICANTS SUBMIT THEIR APPLICATION FORMS AT ANY BRANCH OFFICE OF PHS OR VHSO IN THEIR RESPECTIVE STATES.

    After receiving the applications, PHS or VHSO conducts verification regarding the applicant's educational qualifications and physical health.

    They then call\\/deliver the medical report(s) to the applicant's address within 15 days.

    After completing this process, PHS or VHSSO forwards the application with all required documents to New Delhi for review.

    Once in Delhi, PHS experts conduct medical audit on all submitted applications and documents.

    They then analyze each application and issue a response letter or rejection letter to the applicant via post or email.

    If accepted, PHS arranges required citizenship documents such as passport and birth certificate for their Indian citizens through Indian diplomatic missions abroad.

    A medical card is a travel document issued to an Indian citizen by the Government of India.

    It's valid for 10 years and contains health information of the holder.

    A medical card allows Indians to receive treatment in a government-owned hospital or to purchase medicine from a government pharmacy.

    A holder must be a citizen of India and should have a valid passport to apply for a medical card.

    A medical card is issued as an emergency measure when required, which explains its relevance and significance in today's times.

    The application form for a medical card is available at all district Passport Offices.

    It consists of two parts: an application form and supporting documents.

    The application form asks the applicant personal information such as name, date of birth, gender, citizenship, address and marital status.

    It also asks educational details such as school qualification, university degree, study period and name of the college\\/University.

    Further, it requests nature of illness\\/injuries with duration, treatment received so far, and other relevant information related to the illness\\/injury.

    A medical report from a registered Medical Practitioner (medical doctor) is also required along with the application form.

    All supporting documents should be photocopied and submitted with the application form.

    These include passport size photo with white background; proof of citizenship; copy of father's\\/mother's birth certificate; and copy of caste certificate if the applicant belongs to a Scheduled Caste or Scheduled Tribe.

    If the applicant does not belong to either category, there is no need for a caste certificate.

    The application form should be signed by an officer from the local office of Public Health Services (PHS).

    In some states, such as Bihar, these duties are performed by the Village Health Service Officers (VHSO).

    Allowing time for processing, an application for a medical card can take up to one month.
